Provide Housing Case Management to Veterans in a Transitional GPD home

The focus of this position is to provide individual, group and crisis support mental health services to residents of Joshua Station while being an active member of the Joshua Station team.

As a Creek Targeted Case Manager, you will have a combined role providing intensive case management services to clients at the Creek and traditional case management to clients in Olmsted County.
