Case Manager
Position: Full Time: Salary
Job duties include but are not limited to:
1. Assist in interviewing potential new residents
2. Meet with new resident to do intake documents and be sure they understand all the documents they are signing pertaining to program policies and procedures
3. Within the first week the Individual Service Plan (ISP) will be completed, short term and long term goals will be identified. Barriers to housing will be identified and goal accomplishments will be determined by priority and documented. ISP’s will be reviewed and redone every quarter for each resident.
4. Weekly meetings with each resident will identify what has been accomplished for the week and clear expectations for the following week will be discussed with the resident. Keep all weekly progress notes and documentation on meetings in Residents binders
5. Attend all weekly team staff meetings and coordinate with the Housing Case Manager and Managing Director, to discuss housing barriers to be addressed during that week’s case management. Goals for each resident will be identified to be completed each week and by whom for each resident.
6. Keep all documentation up to date in resident’s binders, to be reviewed by the Liaison upon request
7. A monthly team meeting will be done with both the Case manager, the Housing Case Manager, and the resident to assure goals are being meet and on track. If the resident is struggling, solutions will be discussed.
8. Meet weekly with staff and the VA GPD Liaison to review the selected residents for that week. Goals to be accomplished in the following weeks will be documented.
9. Attend morning house meetings
10. Prepare Monthly Case Management reports for the Board
11. Attend TVPI events and or fundraisers when needed
Qualifications:
1. Must have a BS in Mental Health field and 2-5 years’ experience with a working with Veterans and or homelessness, be understanding and compassionate with a multitude of mental health and or substance abuse issues
2. Confidentiality for residents is a must
3. Strong work ethic and a desire to succeed
4. Must be flexible and able to work Monday through Friday (we are flexible on this)
5. Must be able to work as a team.
6. Clean background check and reliable transportation
7. Good communication skills and be able to multi task
Compensation/Benefits:
- Annual salary is $48,000.00 per year, paid bi-monthly;
- Medical insurance is provided on a cost share bases, 60% provided by TVPI; 40% provided by employee; the medical insurance plan is for one employee only; this option may be waived, if employee is qualified for Veteran Administration medical coverage, or other commercial medical insurance coverage;
- Eligible vacation is based on years of service using the following formula:
First Year - one week per year;
Second Year – one additional week;
Third year and thereafter vacation time accrues at 4 hours per pay period for a maximum of 30 days per year;
- Medical leave is unrestricted, but must be validated by a medical doctor.
- Sick leave accrues at 2 hours per pay period, per year. When accrued sick leave reaches 30 days, more time will not be accrued.
- Family Medical Leave is authorized but must meet the required guidelines and authorization.
Reporting:
The Case Manager reports to the Managing Director.
