- Provides on-site brief intervention, de-escalation and crisis management and recovery support at designated LIHI-operated Tiny Home Village.
- Becomes a visible, stable and accessible presence at assigned Tiny Home Village.
- Through progressive engagement, works with Tiny Home Village residents to identify and begin to address mental health and substance use issues which may present barriers to long-term, stable housing.
- Collaborates with on-site LIHI staff to respond to resident incidents and support resident compliance with Village rules and standards of behavior.
- Offer individual and group counseling sessions while residents are in ideation (pre-enrollment) phase.
- Performs Mental Health intake intakes/evaluations and collects and processes psycho-social data from residents for purposes of clarifying treatment goals and establishing preliminary diagnoses that may be required to access housing or other services.
- Provide a warm handoff to THS staff for residents who wish to pursue formal Mental Health or Substance Use Disorder Treatment.
- Ensures prompt, accurate clinical documentation including treatment plans, progress notations in assigned participant files; preparation of treatment contracts, letters, referrals, discharge summaries of terminated participants as appropriate.
