This is your full name as you would like it displayed on your online course certificate. Please leave out titles and suffixes in this field (Use the "Title" and "Suffix" fields in this name group of fields to manage these).
This is an optional field to display your title for your name. For example if you are a doctor you could enter "Dr.". Limit of 10 characters.
Please enter your first name.
Please enter your last name or surname.
This is an optional field to add multiple suffixes to your name. For example "J.D.", "M.D.", "D.O.", "D.C.", or "Ph.D." Limit of 10 characters per suffix. Multiple are allowed.

This information is part of a uniform data collection tool is used by all SAMHSA grantees as part of the Government Performance and Results Act (GPRA). Only this demographic data and any training evaluations you complete will be shared with SAMHSA. Your name and email address are stored securely in our system and will not be shared outside of this system.
